The Fulton County Office of Aging houses the information and referral component of the Fulton County Aging Network. This is the entry point for all county based aging services ( xception-senior centers) within Fulton County. Case Management is designed to assist older adults and their families determined needed services and provision of needed services. Case Managers are liasons between participants and community based services. All eligible partnerships must have a face-to face interview with the case manager in order to receive in-home services.
Home Delivered Meals (HDH) provides low cost nutritious balanced meals to homebound seniors to facilitate independence and wellness.
Homebound seniors that have experienced deteriorated health or are at risk of placement into long term care facility may be eligible for in home services. In home services provide assistance to seniors in their home with household tasks and personal grooming. These services are Homemaker, Personal Care, and Respite Care.
Eligible seniors are provided with transportation to and from senior activity centers and medical appointments. Fulton County transportation service is a non-emergency medical transportation to participants of South Fulton County
