This information is forwarded to our agency if it is among our servicing area for Case Management Services. A Case Manager will contact individual for an appointment to visit within the home and complete the necessary paperwork.
Transportation Services provide door –to—door pick-up to and from the participant’s home.

Can be provided 7 days a week, 24 hours a day.
Contact or visit the Senior Center near you. To be eligible for assistance, you must be a registered participant of a Senior Center.
If you are requesting Transportation Services as a new participant the Center Manager will complete and process the necessary paperwork. You will be asked to commit to a schedule of Center attendance to pre-schedule your transportation services.
The Center Manager will then contact you to confirm a starting date for your transportation services.
