Case Management

Skilled Case Managers assist older adults and their families in determining the services needed that allow them to safely stay in their homes.  Case Managers are the liaison between the participant (client) and community-based agencies.

In-Home Services

Case Managers assess and certify seniors for in-home care from Certified Nursing Assistants that can include personal grooming, light household tasks, escort service, and essential shopping. 

Transportation Services

Case Managers assess and certify seniors to receive assistance in coordinating transportation through Fulton County Department of Senior Services for non-emergency medical transportation.

Congregate Meals

Seniors attending neighborhood Senior Centers can enjoy a hot nutritious meal five days per week while socializing with others in a relaxed setting.

Home-Delivered Meals

Volunteers deliver low-cost, nutritious meals to frail, isolated and/or disabled adults.
